Job Requirements
What You’ll Do
- Deliver skilled nursing care in patients’ homes
- Perform comprehensive assessments and develop individualized care plans
- Coordinate care with physicians, therapists, and interdisciplinary team members
- Educate patients and families to promote independence and wellness
- Monitor progress and adjust care as needed
- Maintain accurate, timely documentation
- Serve as a trusted advocate for your patients
What You Bring
- Graduate of an accredited nursing program (Diploma, ADN, or BSN)
- Current, active Texas RN license
- Minimum 1-year acute care experience (home health preferred)
- Current CPR certification
- Strong clinical judgment and critical thinking skills
- Excellent communication and interpersonal skills
- Reliable transportation and valid Texas driver’s licens
