Job Description

The Care Specialist will play a vital role in supporting clients' needs by providing compassionate care and exceptional customer service. This role requires effective communication, patience, and empathy to respond to client needs and offer personalized solutions. The Care Specialist is also responsible for documentation, follow-up, and ensuring a supportive, safe environment for clients and field employees.

Key Responsibilities

  • Client Support and Care: Provide direct assistance to clients in various settings, addressing their personal, emotional, and physical needs in a respectful and supportive manner.
  • Communication: Engage actively with clients and families to understand their needs, answer questions, and provide information or updates regarding care plans.
  • Documentation and Record Keeping: Maintain accurate, up-to-date records on client interactions, assessments, and care provided, ensuring compliance with privacy policies.
  • Coordination of Services: Work with other healthcare providers, agencies, and team members to coordinate client services and ensure continuity of care.
  • Problem-Solving: Address client concerns and escalate issues as needed to provide timely resolutions and improve client satisfaction.
  • Monitoring and Safety: Regularly monitor clients' condition, report changes to healthcare professionals, and ensure clients' safety in their environment.
  • Education and Guidance: Provide information to clients and their families about services, resources, and best practices for care and well-being.

Job Requirements

 

Qualifications

  • Education: High school diploma or equivalent; certification or degree in healthcare, social work, or related field preferred.
  • Experience:2yersr pulse previous experience in healthcare, customer service, or care support role preferred.
  • Skills:
    • Excellent communication and interpersonal skills
    • Strong problem-solving abilities
    • Empathy, patience, and a caring attitude
    • Organizational and documentation skills
    • Excellent CRM management and skills
  • Technical Knowledge: Familiarity with medical or healthcare terms may be required depending on the setting.

Additional Information

Additional Requirements

  • CPR and First Aid certification may be required.
  • Background check and valid driver’s license if applicable for home visits.
  • Ability to work flexible hours, including weekends and holidays, if needed.
