Job Requirements

We are looking for Case Manager / Case Management Registered Nurses who combine a true passion for providing the best nursing care, with a demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures. In addition, you should be team-oriented and compassionate professional committed to always providing exceptional service. It is also important that you are highly organized and that you demonstrate excellent communication and interpersonal skills, along with the ability to work well with a diverse population.

Case Manager / Case Management Nursing Duties

The primary goal of a nurse case manager is to ensure that every patient in their care receives exceptional care for your facility, agency or company. To accomplish this, nurse case managers usually perform the following duties:

• Assessing new patients by gathering information, reviewing diagnoses and analyzing medical test results
• Developing complete plans of care to address the needs of patients
• Educating patients about their care plan and health care options and helping them make key decisions
• Scheduling appointments for doctors’ visits, therapies and other medical services
• Revising care plans based on patients’ progress and input from physicians and other medical providers
• Updating patient medical records after each contact
• Offering emotional support for patients and their families

RN Case Manager Responsibilities:

• Conducting intake assessments with patients and families.
• Ensuring that families feel sufficiently equipped and supported.
• Developing individualized treatment plans that account for physical, financial, and other constraints.
• Adjusting treatment protocols, as needed.
• Suggesting and coordinating appropriate follow-up services to promote wellness.
• Ensuring that patients have transport to and from their homes and all pertinent healthcare facilities.
• Meeting with members of healthcare teams to discuss patients' progress, and to deliberate on appropriate care.
• Checking to see whether patients have sufficient insurance coverage or finances to pay for their treatment.
• Devising innovative solutions or suggesting appropriate referrals for patients who cannot afford to access treatment.

 Open Contracts below: 

• Case Manager RN for 13-week Contract in Houston: Day shift: 40 hours a week, Complete initial and ongoing discharge planning assessments, complete final discharge disposition and other regulatory required documentation in the Cerner ACM program. Coordinate referrals and ensure authorization, as required, for post-acute services and transportation.

Specific qualifications for this Med Surg nursing role include:

• Minimum 2 year of Case Manager / Case Management experience
• Current and valid BLS certification
• ACLS certification (if required by client)
• Ability to work in a fast paced, high-risk environment

• Must meet Pulse Staffing’s hiring criteria

• Travel experience, a plus

An RN Case Manager must be able to multitask and handle numerous responsibilities throughout each day. They must also be effective communicators with patients, families, doctors, and insurance providers; capable of understanding and explaining the features of Medicare, Medicaid, and other insurance options. In-depth knowledge of health care services and medical terminology is a requirement as well.
